The goal of transfeminine bottom surgery is to transform the male genitalia and reconstruct it into that of a female. We typically perform transfeminine bottom surgery at our Las Vegas practice as a single-stage procedure. Many techniques are used to perform bottom surgery, and the most common is penile inversion vaginoplasty. VPSI is currently the only practice in Nevada that offers this procedure.
The Benefits of Male to Female Bottom Surgery
If you were born with masculine genitalia but self-identify as female, vaginoplasty can be an essential step in the transition journey. Some benefits of the procedure include:
- Gaining physical features more closely aligned with your self-identified gender
- Achieving more self-assurance and confidence
- Realizing positive mental health
Am I a Good Candidate for Vaginoplasty?
You may be a good candidate for vaginoplasty if you were assigned male at birth, and have gender dysphoria due to your genitals.
Our practice takes pride in being a welcoming and safe space for members of the transgender community, no matter where you are in your transition.
For bottom surgery, we follow wpath guidelines, which means that to be considered for genital surgery, you must have notes of clearance from at least two different mental health professionals.
Taking the First Steps
The initial step in gender affirming surgery is meeting with Dr. Brosious, who is skilled and experienced in performing transmasculine and transfeminine procedures.
During your consultation, he will perform a physical examination, review your medical history, and collect some medical images, all of which are used to ensure you are eligible for a safe procedure.
Additionally, the consultation is an excellent opportunity to ask any questions you may have about the surgery and clarify the specifics of your treatment plan.
Meet Our Board-Certified Plastic Surgeons
At VPSI, our surgeons distinguish themselves by their excellent technical skills and artistic eye. Both Dr. Brosious and Dr. Goldman have the versatile experience that allows them to say “yes” to nearly every patient who walks through the door.
Dr. John Brosious is a full-scope plastic and reconstructive surgeon offering head-to-toe surgeries. His background includes experience in Level I Trauma centers and academic institutions, equipping him to take on the most advanced reconstruction cases. Dr. Brosious performs over 200 chest surgeries a year for both transfeminine and transmasculine patients, and he is also the only surgeon in Nevada performing male to female genital gender affirmation surgery.
Likewise, Dr. Joshua Goldman is a comprehensive plastic and reconstructive surgeon, well-known in the breast cancer community for his specialization in breast microsurgery. He single-handedly has the highest number of microsurgery cases in the State of Nevada and has been named by Vegas Inc’s Las Vegas Weekly as “Las Vegas 40 Under 40” and a “2021 Top Doctor.”
To Dr. Brosious and Dr. Goldman, you are family, and they are here to help guide you through your journey to enhance your natural beauty.

Transfeminine Bottom Surgery Explained
Our Las Vegas practice is the only practice in Nevada performing vaginoplasty or penile inversion vaginoplasty. In fact, Dr. Brosious is one of just a handful of surgeons performing this operation in the country.
Vaginoplasty is a very elegant surgery involving the creation of a female vulva and vagina from a penis and testicles. The vagina is fully functional, and most of our patients can have sex and orgasm after their operation. The results are aesthetically pleasing, meaning the scars are virtually imperceivable, and the results look very natural.

Recovery and Aftercare
Following penile inversion vaginoplasty, you will need to allow your body plenty of time to rest and heal. This means avoiding strenuous exercise for at least six weeks and avoiding bathing, swimming, or submerging in water for at least eight weeks. Due to the extensive nature of these procedures, it is advised to take up to two months off from work for recovery.
